Overall Therapist of the Year 2015 – Hannah Jones

All entrants into Best Young Hygienist/Therapist & Best Hygienist/Therapist will be automatically entered for this award. Judges will consider the following:

  • Demonstration of hard work and drive
  • Any hurdles you have overcome
  • How you have shown you are different from other professionals in your field
  • Training/development
  • How you have shown you go beyond the regular duty of care
  • How you have shown how you connect with the local community
  • Patient testimonials
  • Supporting evidence and pictures.

You can enter as many categories as you like. Simply call the awards team on 01923 851 779 to register your details. The entry fee is £20+VAT per category, however, pre-register your entry by the 4 September 2015 and pay just £10+VAT per category.

Send your entry in by the closing date of 2 October using an online file sharing service such as Dropbox or Wetransfer.com or by uploading your entry onto a USB stick and posting it to DH&T Awards, FMC Hertford House, Shenley Lane, Hertfordshire WD7 9AB.
